Before you call

When you call to set your appointment, I will need your insurance information:

Member ID number
Phone number for providers to call
Plan name, if on your card
Payer ID (a 5-digit number), if on your card

You may want me to coordinate care with another provider, such as your psychiatrist, primary care physician, etc. If so, please complete this form to authorize release of behavioral health information.

Before your first appointment

First appointments last from 1-1/2 to 2 hours; please fill out these forms prior to your session (allow 45 minutes to complete):

As a client, you are entitled to a copy of Healing & Wellness Counseling's Privacy Practices as well as the No Surprises Act Disclosure Notice.
